BridgeCare is seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) who is fluent in Gujarati or Hindi to provide compassionate, high-quality home visit care throughout the Greater Chicagoland area (Chicago + surrounding suburbs). Compensation: Competitive and based on experience ( per-patient/per-visit model ) + mileage reimbursement + malpractice insurance provided .

Duties

• Conduct comprehensive patient assessments during home visits (vitals, medical history, medication reconciliation, focused physical exam) while maintaining high standards of infection control and aseptic technique.
• Provide home-based primary care services including evaluation and management of chronic conditions and common acute complaints.
• Develop and update individualized care plans, including preventive care recommendations and patient/caregiver education.
• Order, review, and interpret labs and diagnostic testing as appropriate; coordinate follow-ups with patients based on results.
• Coordinate care with BridgeCare physicians and other members of the care team; facilitate specialist referrals when needed.
• Document visits accurately and promptly in the EMR, maintaining HIPAA compliance and quality standards. Experience with ICD-10/ICD-9 coding is advantageous.
• Communicate clearly with patients and families in Gujarati/Hindi to ensure understanding, adherence, and culturally sensitive care.
• Travel between patient homes throughout Chicagoland while maintaining organized scheduling and timely completion of visits.

Qualifications

• Active Illinois NP or PA license (in good standing).
• Fluent in Gujarati or Hindi (required) — must be able to communicate directly with patients without an interpreter.
• English documentation required.
• Ability and willingness to travel throughout Chicago and the Greater Chicagoland area ; reliable transportation and valid driver’s license required.
• Strong clinical assessment skills and ability to work independently in patient homes.
• Experience in Primary Care, Geriatrics, Chronic Care Management (CCM), Transitional Care Management (TCM), Internal Medicine, Home Health, Telehealth systems, or House Calls is a plus.
