Job Description

Under the direction of the Program Director, the Social Worker provides a range of services including, recruitment, intakes and outreach of potential participants for the Bayview Adult Day Health Center. This position also consists of providing case management, community care coordination, individual/group counseling and working with families and providers.

Minimum Qualifications

  • Must possess a master’s degree from a school of Social Work.
  • At least 3 years’ experience as a Social Worker, preferably in a geriatric setting or community-based setting.
  • Experience working with the African American community is preferred.
  • Must possess a valid California Driver’s License, have a vehicle and insurance required by the State of California, as well as a satisfactory driving record.

  • Knowledge of resources for elders and their families.
  • Skills in assessing, counseling and group work.

  • Ability to work with participants and their families in a cooperative and tactful manner.
  • Flexibility, creativity and initiative necessary.
  • Registered ASW with the California Board of Behavioral Science.

Essential Duties

  • Develops and implements recruitment, intake and outreach to frail elders and disabled persons who could potentially attend adult day health.
  • Makes presentations to community agencies, health organizations, churches, senior groups and other potential sources of referrals.
  • Attends street fairs, health fairs, special events and neighborhood gatherings.
  • Maintains consistent, close contact with existing and potential referral sources.
  • Maintains an organized system to keep track of referrals from all sources and follows through in a timely manner to arrange for a home visit.
  • Visits seniors in their homes to screen and explain the center's services to caregivers, as well as potential participants
  • Maintains regular contact with potential referrals and follows through on the process to get to the center
  • Obtains a signed medical release form (via mail) and faxes the form and physician treatment plan to the MD, prior to the home visit when possible.
  • Follows through on scheduling TB tests if needed, in a timely manner. Works closely with the MD and ADHC RN.
  • Arranges for a home visit, with the caregiver present, if needed. Obtains signatures for all admission forms, including the para-transit application if needed.
  • Verifies Medi-Cal eligibility or interprets sliding fee sale to participant and/or family.
  • Schedules two - three assessment days with the Office Manager and faxes para-transit eligibility at least 24 hours before the participant's first assessment day.
  • Responsible for social work assessment, reassessments and treatment plans of all program participants.
  • Completes written assessment of social work status prior to the participant's first assessment day, prior to multidisciplinary team meeting.
  • Completes treatment plan within the first three days of assessment.
  • Implements treatment plan based on the needs of participants.
  • Writes quarterly progress notes based on progress in treatment goals, in a timely manner.
  • Ensures documentation of social work services daily through flow sheet for social work services. Writes progress notes in the chart on significant events and changes.
  • Monitors participant's social work needs and arranges for, as well as personally provides appropriate social work interventions to address these needs.
  • Provides referrals to appropriate resources that address the needs of participants and encourages participants to bring social service needs to the attention of the social workers and provides referrals, counseling, peer support and family support.
  • Provides individual and family counseling, including leading support groups.
  • Interprets detailed paperwork relating to medical bills, insurance, finances, etc.
  • Assists participants to maintain their Medi-Cal status and works with Office Manager to assist participants who have problems with their Medi-Cal eligibility.
  • Supervises the work of social work assistants and interns.
  • Assigns case load to social work staff and monitors follow up on each caseload.
  • Designs work plan for social work assistants and assigns group work.
  • Serves as a member of the multidisciplinary team .
  • Participates as the Social Worker in initial assessments, care-planning and re-evaluations of participant's care at meetings and case conferences.
  • Attends weekly multidisciplinary staff meetings.
  • Acts as social work consultant for the staff and acts as primary liaison with the participant's family as appropriate.
  • Acts as liaison with agency consultant psychologist, referring appropriate clients.
  • Encourages effective relationships among staff geared to the building and maintenance of a cohesive team.
  • Other duties as assigned
