Healthcare Navigator Nurse  

• Responsibilities : In this role, you will support patients in managing their healthcare journey by helping them navigate the complexities of treatment plans, insurance, and access to healthcare services. You will collaborate with healthcare providers to ensure that patients receive timely care, explain treatment protocols, assist with appointment scheduling, and provide educational resources on managing chronic conditions. You will also serve as the liaison between patients and the healthcare team, offering continuous support throughout the care process. 

• Required Skills : Strong communication skills, knowledge of healthcare navigation, ability to manage patient records, and proficiency in coordinating services across medical departments. 

• Qualifications : RN with experience in healthcare navigation, patient advocacy, or case management.
