About The Role

The Case Manager plays a vital role in supporting clients as they work toward stability, recovery, and independence. This position requires someone with a heart for serving vulnerable populations, a commitment to meeting clients where they are, and the initiative to step in and support colleagues when needed. The Case Manager will guide clients through individualized service plans, facilitate life skills education, and collaborate with community partners while maintaining thorough, compliant documentation.

Key Essential Duties

  • Complete initial case management assessments to identify client needs, strengths, and service priorities.
  • Develop individualized case management plans collaboratively with clients, ensuring they align with identified goals and needs.
  • Provide ongoing case management services, addressing identified barriers while maintaining flexibility to adjust plans based on client progress.
  • Maintain consistent, collaborative communication with the client’s support system to enhance outcomes and promote stability.
  • Conduct regular face-to-face reviews with clients to assess progress toward service goals, modifying plans as needed to ensure alignment with objectives.
  • Document all service contacts, including face-to-face interactions, family communication, and probation or court updates, in a timely and accurate manner.
  • Ensure complete and compliant documentation of treatment-related information, including case management notes, progress notes, correspondence, and releases of information.
  • Facilitate case management groups, supporting clients in developing skills for recovery and daily living.
  • Actively participate in interdisciplinary staff meetings, contributing to a collaborative, solutions-focused treatment environment.
  • Foster and maintain collaborative relationships with community partners and referral agencies to support client needs.
  • Implement and educate clients on life skills, including money management, time management, medication management, goal setting, communication, interpersonal skills, decision-making, and problem-solving.
  • Take initiative during downtime to support other areas, assist colleagues, and contribute to a positive team culture.
  • Attend required meetings and continuing education to maintain professional growth and ensure best practices in client care.
  • Perform other duties as assigned in alignment with organizational needs, policies, and procedures.

Meetings and continuing education are included as a requirement of this role.

Qualifications

  • High School Diploma or GED required.
  • Minimum of 12 months of experience in substance abuse treatment or a behavioral health-related field; case management experience preferred.
  • Must be at least 21 years of age.
  • Valid driver’s license with no SR22 requirements.
  • Ability to drive clients when necessary to support care delivery.
  • Demonstrated ability to complete accurate and timely documentation in accordance with company policies and state guidelines.
  • Commitment to maintaining a safe, supportive, and accountable environment for clients and team members.

This position is classified as a safety-sensitive position under applicable law. The Navigator role requires alertness and the ability to perform job duties safely and effectively at all times to protect the health and safety of clients, colleagues, and the community.
